I ___________ agree that the payment of $___________ for a membership into the Cremation Society of Alabama is for the services of drafting and finalizing your disposition plan and not towards a pre paid funeral arrangement with the Cremation Society of Alabama.
I also understand that my family or Authorizing Agent is under no obligation to use the participating funeral establishments connected with The Cremation Society of Alabama at the time of my death.
Membership in the Society does not restrict your right to purchase funeral related merchandise and/or services in the open market at any time before death, nor does it restrict the right of your next of kin(s) or your "Authorizing Agent" to purchase funeral goods and services in the open market at the time of death.
